I hereby affirm that I am aware of the inherent risks involved in the aforementioned activity which may result in serious injury or death. I also understand that the description of the risks of the event is not complete and that unknown or unanticipated risks may result in injury, illness, or death. Further, I understand that hiking / trekking / camping/ cycling may be conducted at a site that is remote, either by time or distance or both, from an equipped medical facility. Nonetheless, I choose to proceed with this program at my own volition and sole risk. 

The information I have provided about my medical history is accurate to the best of my knowledge. I agree to accept responsibility for omissions regarding my failure to disclose any existing or past health conditions. 

 

I understand and agree that neither any of the trek/hike/camp personnel coordinating this program nor the entity through which this program is being offered, namely, “JACK AND HILL ADVENTURES TRAVEL LLP” nor its employees, officers, agents or assigns shall be held liable or responsible in any way for any injury, death or other damages to me, my family, estate, heirs or assigns that may occur as a result of my participation in this program or as a result of the negligence of any of the aforementioned parties, whether passive or active. 

 

In consideration of being allowed to participate in this program, I hereby personally assume all risks for any harm, injury or damage whether foreseen or unforeseen, including but not limited to Acts of Gods that may occur. 

 

I further authorize “JACK AND HILL ADVENTURES TRAVEL LLP” to take necessary medical measures in the event of an emergency. I shall reimburse the expenses for the necessary medical measures spent by “JACK AND HILL ADVENTURES TRAVEL LLP“ within 3 working days. I shall not hold them responsible in any way for loss or damage to person or property. 

I agree to abide by the rules and regulations laid down by the “JACK AND HILL ADVENTURES TRAVEL LLP".

I undertake to indemnify “JACK AND HILL ADVENTURES TRAVEL LLP” and make good any loss and/or damage (except reasonable wear and tear) caused to any equipment used or set to be utilized for the aforementioned activity, which is attributable to me / my conduct. 

I agree to "JACK AND HILL ADVENTURES TRAVEL LLP" using my photographs taken for future promotional media
